GE Motor – Model 5K37MN38 – 3/4 hp 3540 RPM, 3 phase 208/460, Frame 56J, main shaft is .625 dia and 1.75” long.  Threaded on the end for 3/8” fine thread about 3/4 ” long.  4 flange mounting screw holes 3/6” x 16.  I believe this motor is new/unused. I have found the following link very helpful in matching NEMA frame specs and dimensions. http://www.engineeringtoolbox.com/nema-electrical-motor-frame-dimensions-d_1504.html  I urge you to use this reference wherever possible to be sure you are getting the right motor.
