One Polycom Soundstructure C16.  Working pull!  I wasn't able to test it fully, but I did reset it to factory defaults, upgrade the firmware to the latest version (1.7.2), and play around with adding channels in Studio, and dialing out from the PSTN card. The C16, has 16 total channels IN or OUT and accepts a wide variety of sources using either the phoenix connectors, or the C-LINK ports (i.e hooking up HDX digital microphones).  This piece of equipment is pretty advanced and well known in the world of audio conferencing, Polycom has much more information on their support site including design documents. This chassis INCLUDES the TEL2 card.  This is a 2-line PSTN card which allows you to connect to 2 standard phone lines. Again, this is a working pull, but is sold strictly AS/IS, no warranties or guarantees of any kind. Its in pretty good shape, some scratches and scuffs from handling and use. GREAT buy-it-now price, this thing was insanely expensive when it was brand new.  Thanks for looking!
