  A very comprehensive list of 2012 ICD9 diagnostic and CPT procedural codes, including the E & M codes for office visits and consultations, for the ophthalmology or optometry practice.  These listings are so complete that they are also suitable for any of the ophthalmic sub-specialties.  Includes the new 2012 ICD9 diagnostic codes for glaucoma, glaucoma severity, eyelid and facial skin malignant tumors, and changes in the migraine headache sub-category codes.  An additional document is included that lists all of the new 2012 ICD and CPT changes pertaining to ophthalmology.  This document includes new codes, deleted codes, and modified codes or code descriptions.  All of the common diagnoses are included, such as:      Corneal abrasion -  918.10      Cataract, nuclear sclerotic -  366.16      Entropion, senile -  374.01      Macular degeneration, atrophic -  362.51      Floppy iris syndrome -   364.81 But this list also includes all of the more obscure diagnoses that are occasionally encountered in ophthalmology, even those from many other areas of the ICD9 database, such as:     Infectious Diseases:      Lyme disease - 088.81; Toxoplasmosis, disseminated - 130.8     Congenital disorders:     Axenfeld's anomaly - 743.44, Crouzon's syndrome - 756.0     Neurology:     Parkinson's disease - 332.0, Myasthenia gravis - 358.00 as well as the more esoteric ophthalmic diagnoses, including:      Vogt-Koyanagi-Harada disease - 364.24      Essential iris atrophy - 364.51         Coloboma, Morning glory optic disk anomaly - 743.57 There are over 1400 ICD9 diagnostic codes listed in all, and many diagnoses are cross-listed in multiple sites, for easy reference.  I reviewed ALL of the categories and ALL of the sections of the entire ICD9 and the entire CPT code databases to compile this comprehensive specialty-oriented listing.  The CPT codes include:      All of the ophthalmic surgery codes (65000-68899), including the recently added new CPT codes 92071 and 92072, the deleted CPT codes, and the previous 2008 - 2011 changes in newer retina/vitreous codes, and the latest 2011 codes for retinopathy screening, amniotic membrane placement and ocualr surface reconstruction, canaloplasty/viscocanaloplasty, and the 2011 changes for coding for OCT/HRT/GDx imaging of the anterior segment, retina and optic nerve, as well as the recent code description change for laser iridotomy.      Ophthalmic diagnostic procedures and eye exam codes (92002-92499) The list also includes appropriate codes from the:      Integument Section (Repair, Excision, Flaps, Grafts, Incision/Drainage, Rhytidectomy, Blepharoplasty)      Musculoskeletal Section (repair of orbital and facial fractures, reconstruction, bone grafts, fascia lata graft)      Otolaryngology section (nasal endoscopy, ethmoidectomy)      Radiology section (ocular A and B scans, pachymetry, and dacryocystography)      Evaluation and Management codes for office visits and consultations (99201-99456)      The newer screening exam codes for glaucoma      Disability examination codes      Injection codes      Botox codes      Vision service codes for contact lenses and eye glasses      Even the modifiers are included in the list.  The ICD9 and CPT codes are all in Microsoft Word format.  The files for the ICD9 codes are created in different font sizes to meet various preferences.  For example, all 1400 or so ICD9 codes fit onto two pages in the smallest font (6 pt font, seven columns per page), to create a single code sheet (front and back), ideal for laminating for use in each exam room and the business office.  If this font is too small for the more presbyopic user, Word files are also included for the entire database in 8 pt (4 pages) and 10 pt (8 pages) font sizes for easier readability.    The CPT codes are in an 8 pt font that is easily readable and occupies 6 pages in total:  four pages of surgical codes (including the modifiers), and two pages of Evaluation and Management and Ophthalmic Procedure codes.  File versions with larger 10 pt font are also included. These files are sent to you after purchase, by email, as a compressed zip file attachment that contains all of the included files listed above, with the various font sizes of the ICD9 and the CPT code lists. This allows you to print up coding sheets for each exam room, the minor procedure room, the front desk, the billing office, your brief case, the surgical center, and create files for your laptop or PDA.  These are much easier for everyone in the office or ASC to use than voluminous books or complex software programs that require expensive updates. All of these files can be read with MS Word, versions 97 or later. Most current email services and even Windows XP/Vista/7 will automatically unzip this compressed file into its individual Word document files as soon as you download them from your email attachment that you will receive from me.
