This is EMERSON K55HXCEP-6404 Furnace Blower Motor 1/3HP 1050RPM 4SPD 115V used part.  This part was removed by HVAC installer from a working unit. Old system was replaced with newer one (more energy efficient) and was in good working condition.  Motor has been tested, works well and it is in good cosmetic condition. Please see more details from the images attached.
