Has the model number 550-04 on the label, but I cannot trace it to its manufacturer. This first-surface mirror is in good to very good condition, but I cannot attest as to any "lab-grade" quality. There is some dust on the mirrored surface.  There are a very few areas where there are some minor rub marks.  Some of the "silvering" (the mirror coating--which is typically aluminum, not silver) is pitted, but only around the edges. It would work well for general hobby uses. (It's hard to photograph this thing correctly, as you might guess--it's a MIRROR!) Size is closer to 300mm x 300mm x 6mm.
