Looks like rare used, working Fisher model 145; heat block/ dri-bath/dry bloc, 120 volts, 9.5 AMP- accept four heat blocks- one that could   accomodate  5 tubes total with diameter 25   mm (50 ml  culture tubes) two block that could accomodate 12 tubes each with diameter 12 mm (1.5 and 2 ml tubes) and one block that could accomodate 20 tubes total with diameter 9 mm (0.5 or 0.65 ml tubes) .
