As mentioned this is a Lennox 1/2 HP variable speed combination motor controller part number 18M8001. New part # is  70W4601.  It was removed from a Lennox  G61MPV -36C-090-07  It is a GE motor model ECM 2.3.  There are two plugs, one for the power and a 16 wire connector for the controls. I removed it because it was thought it was causing the ghosting or hunting of the motor speed. That was not the problem so the motor runs fine. It has very low hours. Bearings seem smooth and no play noticeable on the shaft.  A replacement motor and controller is over $500. Any questions just ask. Bruce
