This appears to be a South Bend factory taper attachment, although I don't see their logo anywhere on the piece.  Someone please correct me if this is not a genuine South Bend attachment.  Most of the paint is gone and the metal is discolored.  Many dings consistent with being in a working environment.  Needs a good cleaning. The bed bracket has been broken and a welded repair is evident.  See photo.  The bed bracket gib has been replaced with a plastic one.  You will probably want to replace that.  Also the slide block gib is missing. I am not a machinist so I am descibing this as best as I can.  Questions are welcome.
