Two knobs- one mounting screw, one locating bracket and one box Beckman Duodial Model RB. A datasheet including dimensions is available at http://www.bitechnologies.com/pdfs/rb.pdf. You attach it to a multi-turn potentiometer or encoder, and the outer dial keeps track of the number of turns the shaft has rotated, while the inner dial shows the percentage of rotation of the current turn.

Here is a nice RCA box probably CTS 1Meg Potentiometer with brass Small 2" Pull type D shaft that operates two types of switches.
